Partially Stable Pelvic Ring Stress Fracture Complicated by Undercorrection after Bilateral Medial UKA – a case report

雙側內側單髁膝關節置換術後因矯正不足併發部分穩定型骨盆環壓力性骨折:一病例報告

Pelvic stress fractures are caused by repeated stresses to the bone. They usually affect the pubic bone and cause exercise-related pain which gets gradually worse, but they do not usually prevent exercise. Medial UKA may fail if undercorrection of the varus deformity. 

A 75 years old female showed up in orthopedic OPD. She walked with a walker aid. The impressive gross appearance was her right varus knee. She complained pain of her right buttock and inguinal regions for one week. She denied falling down before starting of the pain. Long leg x-ray survey revealed bilateral medial UKA and undercorrection of her right knee. Also fractures of right superior and inferior pubic rami was noted. The CT concluded belonging to the category of a partially stable pelvic ring fracture.

The management of her conditions was complicated. Her osteoporosis and severe varus knee lead to a new indication for ORIF of her pubic stress fracture. And the necessity of concomitant revision to TKA of her right knee. After one month follow up, her pubic pain decreased. She still walked with a walker and the right varus knee wasn’t revised. Long term outcome will be followed.
